Many children with chronic illnesses are living longer due to advances in medical treatment.  This proposal will fund a Center for Excellence in Transition Care to aid these adolescents and you adults as they move from pediatric to adult centered medical care.  It will be measured against an adolescent medicine program to see the percentage of individuals that are seen by an adult provider within one year.
